New Patient Registration
If you live within our practice area you are welcome to register with us and our reception staff will be happy to guide you through the procedure. You can also register with us online via the NHS App.
You will need to complete a registration form (GMS1) and a health questionnaire which will provide useful information whilst we wait for your medical records to arrive from your previous doctor. There are practical reasons why [we] might need to be assured that people are who they say they are, or to check where they live. Seeing some form of ID will help to ensure the correct matching of a patient to the NHS central patient registry, thereby ensuring any previous medical notes are passed onto [us]. However, anyone in England can register with a GP Surgery without needing proof of address or immigration status, ID or an NHS number.
All new patients are offered a health check with a member of the healthcare team to ensure that any required tests are up to date and that we have an accurate note of any repeat medication you may be taking.
Each of our surgeries has a full compliment of staff, with onsite dispensary*.
*As a Dispensing Practice there are rules that govern who we can dispense medication to. Anyone who registers and lives within 1.6KM of a pharmacy cannot be registered as a Dispensing Patient at our Surgery and will be Non-Dispensing; this means you will be issued with a prescription which you will need to fulfil at a community chemist. Please therefore take this into consideration when registering with our Practice as we will not be able to provide your actual medication, just the paper prescription.
If we cannot Dispense medication for you, you will need to take your prescriptions to a Pharmacy for these to be dispensed. You can choose to nominate a Pharmacy. At the moment, our practice has not been upgraded to EPS release 2 (Electronic Prescription Service), which means that we cannot electronically send prescriptions to your nominated pharmacy. We do however have relationships with some local pharmacies who pick up prescriptions from us, please ask reception for that information on registration.

The practice will do its utmost to ensure that all patients are treated fairly and the practice environment is free from discrimination and harassment. We can access the following services although prior arrangement may be necessary. *Sign language services.
